*TERMS & CONDITIONS ~ MEDIA RELEASE FORM

This Media Release Form ("Release") is entered into on [Date] by and between the undersigned client ("Client" or "Releasor") and Oakmont Skin Care & Electrolysis, Inc. ("Company" or "Releasee"), located in Oakmont, Pennsylvania.

1. GRANT OF PERMISSION
   For valuable consideration, the receipt and sufficiency of which are hereby acknowledged, I hereby grant Oakmont Skin Care & Electrolysis, Inc., its legal representatives, successors, and those acting with its permission, the absolute, irrevocable, and unrestricted right and permission to take, use, reuse, publish, and republish photographic portraits, pictures, video recordings, audio recordings, or digital media of me, or in which I may be included, in whole or in part, or composite or distorted in character or form, without restriction as to changes or alterations.

2. SCOPE OF USE
   I understand and agree that these photos, videos, and/or audio recordings may be used for any lawful promotional, marketing, or advertising purposes. This includes, but is not limited to, the Company’s online retail website, marketing emails, printed promotional materials, and official social media platforms (such as Instagram, Facebook, TikTok, and YouTube), encompassing both organic posts and paid advertisement campaigns.

3. WAIVER OF COMPENSATION AND INSPECTION
   I hereby waive any right to inspect or approve the finished photograph, video, advertisement, or accompanying printed or electronic copy that may be used in conjunction with my likeness. I further acknowledge and agree that I will receive no financial compensation, royalties, or residual fees for the taking or use of any such media, now or in the future.

4. OWNERSHIP
   I acknowledge and agree that Oakmont Skin Care & Electrolysis, Inc. is the sole and exclusive owner of all rights, title, and interest in and to the photographs, video recordings, and other media captured during the session, including all copyrights therein.

5. RELEASE OF LIABILITY
   I hereby release, discharge, and agree to hold harmless Oakmont Skin Care & Electrolysis, Inc., its owners, estheticians, employees, and legal representatives from any and all liability, claims, demands, or causes of action arising out of or in connection with the use of my likeness, including without limitation any claims for defamation, invasion of privacy, or right of publicity.

I represent that I am at least 18 years of age and have the full legal capacity to sign this document. I have read this Release before signing below, and I fully understand its contents, meaning, and impact.
